La buena fortuna de la otología reside en el hecho de que en la mayoría de los casos se puede establecer un diagnóstico mediante un cuidadoso examen otoscópico: la membrana timpánica es la ventana del oído medio. La otoscopia constituye la primera fase en el examen del paciente. El inicio de un joven otólogo comienza con este paso básico. Los colegas de mi generación recordarán los largos meses de entrenamiento que fueron necesarios para entender e identificar algo en las profundidades de un conducto externo estrecho, tortuoso y sensible, frecuentemente obstruido por secreciones fisiológicas o patológicas. Es difícil encontrar buenas ilustraciones en un libro de texto. Había solamente dibujos y lar- gas páginas de descripción sin apreciación ni comparación con la iconografía sin igual de Politzer o Toynbee en el pasado siglo... Las fotografías también estaban ausentes o cuando estaban incluidas, eran de una calidad mediocre, por tanto presentaban un limitado interés. Experimentamos un sentimiento de frustración en la era del Microscopio electrónico y de sondas espaciales que traen fotos de la Tierra tomadas desde la Luna. Los sistemas ópticos modernos, en especial el microscopio binocular, han permitido un acceso sin trabas y observación detallada de la membrana timpánica bajo condiciones óptimas de iluminación y magnificación. La suma de tubos de observación y cámaras de video han ayudado a nuestra familiarización con las distintas condiciones patológicas. No obstante, la membrana timpánica se ha defendido por sí misma de la intrusión foto- gráfica. Inclinada en relación con los tres planos espaciales, y de un diámetro de 1 cm (mientras el conducto normal acepta solamente un espéculo de 4-rnm) , es solamente a través de una exploración progresiva que observamos la totalidad de la superficie. En español, además de la versión de Cansinos, que ha perdido fuelle en los últimos años por el casticismo un tanto démodé de su lenguaje, contamos con la del académico de la Real Academia de la Historia Juan Vernet, que vio la luz en tres volúmenes de la colección «Clásicos Planeta» y ha sido reimpresa varias veces. Pero no había hasta la fecha una traducción que ubicase Las mil y una noches en el lugar que le corresponde en el mundo hispanohablante del siglo XXI. Ha correspondido llevar a cabo esa labor al arabista Salvador Peña, profesor de la Universidad de Málaga, que ha realizado una tarea titánica para desentrañar todas las claves del texto original, ahora dispuesto en perfecto estado de revista y lectura para las nuevas generaciones. La de Salvador Peña va a ser, estoy seguro de ello, la traducción definitiva de las Noches al castellano hasta el día de hoy, la más precisa, la más fiel y, al mismo tiempo, la más elegante y legible que se haya publicado nunca en la lengua de Cervantes. La he conocido desde el comienzo de su gestación, y me consta su probidad insuperada en todos los aspectos que rodean una versión. Hasta los numerosos versos que jalonan, aquí y allá, el original árabe han sido objeto de una versión métrica en español por parte del profesor Peña, que no ha vacilado a la hora de entregarse por completo a un trabajo que supone un eslabón importantísimo en la cadena de las traducciones de las Noches a nuestra lengua. La versión de Salvador Peña es, para mí, la consecución de un deseo largamente acariciado y nunca satisfecho del todo antes de su labor. Las mil y una noches merecían una versión como la suya, que rinde culto al texto original sin traicionarlo nunca, pero que nos ofrece la posibilidad de acceder a él en un español niquelado y diáfano, de nuestros días y de siempre, pendiente siempre de reflejar la desnuda oralidad del relato árabe y, a la vez, atento al adorno retórico cuando este existe en su modelo. No me queda más que felicitar al brillante arabista andaluz que ha sido capaz de organizar una fiesta lingüística tan hermosa y tan perdurable, y felicitarme por haber tenido la feliz oportunidad de participar en ella activamente con estas breves y entusiastas líneas preliminares.

Sin duda, la cirugía ha constituido, y constituye, uno de los pilares fundamentales del saber médico, pero en los últimos años los programas impartidos a los alumnos se han modificado en cuanto al número y la profundidad con que se tratan los temas. Los libros que en la actualidad manejan los estudiantes de habla hispana son, en su gran mayoría, traducciones de textos norteamericanos cuyo contenido no se adapta a lo que se les exige a nuestros alumnos. Por otro lado, aunque ha habido excelentes libros de cirugía en nuestro idioma, en la actualidad la mayoría de ellos han quedado obsoletos.

Hoy el estudiante necesita un libro en el que se expongan con claridad los principios básicos de la cirugía que debe conocer cualquier médico, que resalte las ideas fundamentales a fin de fijar conceptos claros y útiles, sin datos innecesarios, con una extensión limitada, con una iconografía que facilite la comprensión del texto, y con una bibliografía que sirva a quien quiera profundizar en una materia concreta. La estructura clásica del libro dirigido al discente ha cambiado radicalmente. Es necesario introducir en cada capítulo los objetivos por conseguir, resaltar los conceptos más relevantes y elegir cuidadosamente una iconografía clara y que sirva para hacer más comprensible el texto.

Se creó un sitio web complementario en que se ofrecen materiales didácticos como videos de operaciones y maniobras quirúrgicas, imágenes adicionales y más de 700 preguntas de autoevaluación.
